Through a detailed assessment process with their Care Manager, clients and their families gain an objective perspective on their situation. Care Managers develop a plan of care that addresses identified needs and improves quality of life while anticipating developments as the client’s situation evolves. Clients may choose to utilize care management services alone or in combination with Home Care Aide services.

In the event of a sudden change in an aging parent’s status, families are reassured that a knowledgeable expert is ready and able to respond 24 hours a day, 7 days per week.
What Is an Aging Life Care Manager?
An Aging Life Care Manager, also known as a Geriatric Care Manager, is a health and human services specialist who acts as a guide and advocate for families who are caring for aging parents, older relatives, or disabled adults. Our Care Managers are educated and experienced in any a variety of fields important to home care services, including gerontology, nursing, social work, psychology, and occupational therapy, and have specialized focus on issues related to aging and elder care.
All our Aging Life Care Managers are members of the Aging Life Care Association® (ALCA) and meet stringent education, experience, and certification requirements of the organization. All members also must adhere to a strict code of ethics and standards of practice.
