Care management services at Windward Life Care start with an introductory meeting with an Aging Life Care Manager®, your partner in aging well. Through a detailed assessment process with their care manager, clients and their loved ones gain an objective perspective on their situation. Care managers work with clients and those who support them to develop a plan of care that addresses identified needs and improves quality of life, while anticipating developments as the client’s situation evolves. Clients may choose to utilize care management services alone or in combination with home care aides and/or home health services.
The professional guidance of a care manager is invaluable for long-distance loved ones, working families, and anyone who needs support navigating the maze of the health care and long-term care systems. In the event of a sudden change in an aging person’s status, they can be reassured that a knowledgeable expert is ready and able to respond 24 hours a day, 7 days per week.
An Aging Life Care Manager, also known as a geriatric care manager, serves as a personalized guide and advocate for older and disabled adults, as well as those in the support system. Our care managers are educated and experienced in a variety of fields including nursing, social work, rehabilitation, mental health, and dementia care, and have specialized training on issues related to aging and disability.
