EXAMPLES OF DUTIES:

  • Assist with bathing, dressing, and grooming of client on a routine basis.
  • Assist with toileting and incontinence issues of client as needed.
  • Provide stabilization and assistance with walking as needed, including transferring client to bed, chairs, wheelchair, etc., as required.
  • Manage dementia and Alzheimer’s related behaviors, (i.e., wandering, paranoia) as required.
  • Prepare meals and clean-up meal-related items.
  • Provide medication reminders and appointment reminders.
  • Perform light housekeeping tasks such as dusting, vacuuming, making beds, cleaning bathrooms and kitchens, and laundry.
  • Accompany client to personal and medical appointments.



REQUIREMENTS
:

  • One year of in-home care experience with elderly individual; or equivalent experience and education in a CNA program.
  • Must include experience with activities of daily living (ADLs), including incontinence care.
  • Experience working with client(s) with dementia
    – Bathing, dressing & feeding a person with dementia.
    – Experience managing difficult behaviors.
    – Experience with activity engagement
  • Ability to pass background check.
  • Must possess your own reliable, safe, fully-insured vehicle along with a valid driver’s license and current auto insurance. Ability to drive or take public transportation to client locations is also required, and providing incidental transportation using your vehicle or a client’s vehicle may also be required.
  • Able to perform activities of daily living (ADLs) (i.e. grooming, toileting, bathing)
  • Ability to lift, push or pull up to 40 lbs. unassisted.
  • Ability to follow safety precautions and procedures.
  • Ability to work at least one weekend day per week.

Windward Life Care is seeking a part-time Staffing Coordinator to join our team! Responsibilities: The Staffing Coordinator works with the other members of the Staffing team to schedule our 100+ Home Care Aides (HCAs) on an ongoing basis. This includes being responsible for the following in a time-sensitive environment:

  • Matching the skills and availability of our HCAs with client needs
  • Using staffing software to make sure all shifts are filled
  • Managing call-offs
  • Answering daily communications from HCAs and Care Managers
  • Verifying hours worked for payroll

In the uncommon event normal Home Care Aide coverage cannot immediately and appropriately be scheduled, the Staffing Coordinator may also travel to client homes to perform direct client care services until a regular HCA can be scheduled.

Work Schedule: The regular hours for this position are part-time 24 to 25 hours per week – Monday through Friday between 8:30am to 5pm. With the potential of increasing to full-time hours in the future to meet customer demands. On-call, after-hours staffing support is also needed on a rotating basis. This position will be on-site working in one of our corporate offices – San Marcos or Downtown San Diego for the initial 60 to 90 days. Based on job performance, there is a possibility of transitioning to a hybrid remote schedule.

Desired Knowledge, Skills & Experience Include:

  • Highly desired: 1+ years of Staffing/Scheduling experience in a home care/home health setting
  • Excellent communication and collaboration skills
  • Ability to successfully and creatively problem solve
  • Ability to remain calm and focused, multitask, and maintain a sense of urgency in a fast-paced environment
  • Ability to be detail-oriented when dealing with a large volume of incoming information
  • Ability to maintain confidentiality and a high level of professionalism
  • Demonstration of strategic, organizational, analytical, communication, and judgment skills
  • Strong technology proficiency, including use of web-based office systems such as Google Workspace – experience with ClearCare is a plus
  • Ability to pass Home Care Services Bureau (HCSB) background screen

The RN or LVN Case Manager manages the Company’s after-hours clinical concerns as referred or requested. The majority of the on-call hours for this position are expected to take place after-hours during the weekdays:

  • Mondays from 6:30AM through 8:30AM
  • Mondays, Tuesdays, Wednesdays, and Thursdays, from 5:00PM through 8:30AM

Responsibilities include:

  • Manages crisis situations
  • Handles urgent client and staff concerns
  • Oversees and assures quality of care provided to clients after hours
  • Orients Home Care Aides to new client needs
  • Completes skilled nursing visits in 1:1 settings, including medication management, injections, ostomy and catheter care, g-tube feedings and other nursing procedures as clinically necessary
  • Assesses and manages client risk
  • Partners with On-Call Staffing Coordinator to communicate staffing and schedule changes to clients and involved parties
  • Handles calls from prospective clients
  • Advocates for clients currently in home, facility and medical settings, by phone or in person, depending on need and on public health orders.

Desired Knowledge, Skills & Experience include:

  • Active California Vocational Nurse License (LVN) or Registered Nurse (RN) License
  • 2+ years of paid clinical nursing experience with adults who are older and/or have physical and/or mental disabilities
  • Excellent written and verbal communication skills with attention to detail
  • Working knowledge of the provision of physical and mental health care in a variety of settings
  • Working knowledge of care resources, insurance and care delivery systems in San Diego County
  • Moderate+ technology proficiency, including use of smartphone and web-based office systems such as Google G Suite
  • Ability to work independently and manage multiple priorities in a fast-paced environment, and report mileage, expenses, and billable hours accurately
  • Active driver’s license, clean driving record, and reliable vehicle
  • Current CPR certification
  • Ability to pass a drug screening and health assessment
  • Ability to pass a Home Care Services Bureau (HCSB) background screen and TB test
  • Must be fully vaccinated for COVID-19 including booster (if eligible) or eligible for medical or religious exemption per CDSS requirement
